Form submissions are delivered and stored through trusted third-party services (currently Formspree) that process the data on our behalf. A copy may also be saved temporarily in your own browser as a backup. Access to submissions is limited to the Black Healthcare Advocate team.

The questionnaire is for product research, not medical advice, and it is not a medical record. Please don't submit detailed clinical or sensitive health records through these forms. As a pre-launch project, this website is not a HIPAA-covered service; we'll update this policy with full details as our product and protections evolve.
